Cancellation Policy
Client Waiver and Release of Liability A Wellness Collective, LLC This waiver is entered into by the (“Client”) for participation in yoga, somatic movement, meditation, and sound therapy sessions (“Services”) provided by A Wellness Collective, LLC. (“Provider”). 1. Assumption of Risk Client understands that participation in the Services involves physical and emotional activities that may carry risks, including injury or distress. Client assumes full responsibility for these risks. 2. Medical Disclaimer The Services are not a substitute for professional medical care. Client is responsible for consulting a healthcare provider before participating. Client will notify the Provider of any medical conditions or limitations. 3. Release of Liability Client agrees to release and hold harmless the Provider and its instructors from any claims or liabilities arising from participation, except in cases of gross negligence. 4. Code of Conduct Client will follow all instructions, notify the instructor of discomfort or pain, and respect the guidelines provided during sessions. 5. Confidentiality and Consent Personal information shared during sessions will remain confidential unless required by law. 6. Cancelations and No-shows: If a client has been scheduled and cancels appointment within 1 hour of appointment time or no-shows for the scheduled appointment, a $15 fee will be charged. 7. Acknowledgment Client affirms they are of legal age, have read this Waiver, and agree to its terms.
